After you call to schedule your appointment, you will be asked to complete a detailed medical history form online through our secure website. This allows Dr. Pujari to review your medical information to ensure that she understands your health concerns as much as possible prior to your arrival. View our privacy policies.

If you have medical records that you wish Dr. Pujari to review, it is very important that you have them forwarded to the office by the time of the appointment. Dr. Pujari will review them in detail. If possible, please also bring a copy of your records to the appointment. Download forms to obtain medical records from your doctor.

Please be sure to bring all bottles of herbs, vitamins, supplements and medications that you are using to your appointment. The names alone are insufficient for Dr. Pujari to evaluate their impact on your health.
