Our policy generally limits disclosure of your health information to you alone and those family members and health professionals that you have specifically identified. Prior to your visit, we ask you to complete on-line health forms for your convenience. These forms are submitted over our secure web site. The information contained in them is protected from others inadvertently or intentionally viewing them. For security reasons we will not share your records with you over the internet. Our Secure Patient Registration Web Site
Our patient registration web site is located on a separate site from our general information site to ensure security. It is located at https://www.pujaricenterforms.com. The security of all information that you submit is managed with secure socket layer technology. Upon opening our secure site you may see a warning box that indicates that you are about to enter a secure site. The warning box will list some information about the Web Server Certificate that authenticates our site. This certificate authenticates the site as being owned and managed by us and therefore safe.
